Surgical Pre-Screening Package

This package is required for your pre-admit appointment that will be held at Sault Area Hospital approximately a week before your surgical date.

We do provide a copy of this form at the time your sign surgical consent in Dr. Fraser's office, however many patients sign consent at the hospital clinic and may need to download this resource.

If you are are pediatric patient please be sure to choose the pedaitric option.
